Job Title :                            Care Manager – Home Care Services – (Monday – Friday) 

Reporting to :                     General Manager

Location :                            East Limerick – some duties across regions

Overview :

Provide homecare services to help people live independently in their homes. Also operate Irelands first purpose built community for people living with dementia.

          

Responsible for :

The co-ordination and supervision of caring activities carried out by the staff as directed by the Board aimed at providing care for older persons in their own home. The management and development of the individual care plan for the Service Users and management of staff as assigned by the HSE and individual Service Users.

 

Key Responsibilities

  1. Co-ordinate and manage the implementation of planned care for Service Users including:
  • Implement care plan for each Service User
  • Co –Ordinate care for Service Users
  • Liaise with Client Care Co-Ordinator
  • Interview caring staff as per company procedures
  • Delegate Caring duties to HCA
  • Monitor staff caring duties
  • Manage reviews with Service Users
  • Maintain Service User confidential records
  • Liaising and networking with statutory, community and other bodies as appropriate
  • Liaising with Service Users
  • Promote the services of the company in the area to both new and existing Service Users
  • To improve access to and maintain existing services to the elderly and other members of the community
  • Identify methods of complementing the service
  • Administration for Care Plan and Care Support Workers to include the following;
  • Control and monitoring of staff rosters as per staff plan
  • Recruitment and induction of employees
  • Completion of individual Care Plan
  • Implementation of staff performance reviews with employees with a view to encouraging and motivating employee morale
  • Production of monthly reports for managers meeting
  • Organise effective team meetings
  • Manage and maintain monthly budget

 

Benefits :

Salary : €50,000 negotiable depending on experience 

10 days sick leave

23 days  Annual Leave
