PE Global Healthcare are currently recruiting for an Clinical Nurse Manager for a Client based in Leopardstown. 

As an Clinical Nurse Manager you will play a crucial role in delivering specialised care to those who need it most. The CNM will work as part of the Patient Flow team supporting and deputizing for the ADON, Patient Flow

What the role entails:

• Receive patient referrals from the key referral sources, including the acute hospitals; review referrals and liaise with the referral teams / Discharge Coordinators.
• Completion of a pre admission assessment to ensure the referral will be suitably placed in the Care Centre and ensure they are suitable for the service they have been referred for.
• For short term and rehabilitation admissions the CNM will work with the ADON to ensure that there is a discharge plan and anticipated date of discharge set.
• Act as case manager for patients as required and as agreed by Patient Flow

• Knowledge and awareness of HIQA standards (Safer, Better Healthcare).
• Ensure all admissions to the Centre are carried out correctly and all appropriate documentation completed.
• Attend daily handovers and weekly MDT meetings.
• Liaise with doctors about admissions, prescriptions and ensure admission paperwork is prepared and available for the admitting doctor.
• Follow-up on patient’s outpatient appointments if necessary
• Refer patients to Medical Social Worker as required.
• Ensure effective communication with all referral sources and discharge agencies in the community as required.

Requirements

• BSc in General Nursing or equivalent.
• Hold a recognised Managerial Qualification
• Clinical experience in a residential setting
• Registered on the NMBI live register
• Excellent Leadership, Organisational and Communication skills.
